PARTICIPANT WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNIFICATION AGREEMENT THIS IS A LEGALLY BINDING AGREEMENT. READ CAREFULLY BEFORE SIGNING. In consideration of being permitted to participate in the football camp, tournament, combine, or related event (the "Event") organized by the host institution and/or its affiliated program (collectively, "Organizer"), the undersigned participant and/or parent/legal guardian of a minor participant ("Participant") agrees to the following terms: 1. ASSUMPTION OF RISK Participant acknowledges that participation in football activities involves inherent risks of injury, including but not limited to sprains, fractures, concussions, paralysis, and death. Participant voluntarily assumes all such risks, whether known or unknown, including risks arising from the negligence of the Organizer, its officers, directors, employees, coaches, volunteers, agents, sponsors, and affiliated entities (collectively, "Released Parties"). 2. RELEASE AND WAIVER OF LIABILITY To the fullest extent permitted by applicable law, Participant hereby releases, waives, discharges, and covenants not to sue the Released Parties from any and all liability, claims, demands, actions, causes of action, costs, expenses, and damages of any kind or nature, in law or in equity, arising out of or relating to participation in the Event, including claims of negligence, gross negligence, or breach of duty by the Released Parties. 3. INDEMNIFICATION AND HOLD HARMLESS Participant agrees to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or resulting from: (a) Participant's participation in the Event; (b) any breach of this Agreement by Participant; or (c) any act, omission, or negligence of Participant. 4. MEDICAL AUTHORIZATION Participant authorizes the Released Parties to obtain emergency medical treatment on Participant's behalf if Participant is unable to provide consent. Participant acknowledges responsibility for all medical expenses incurred. Participant represents that they are physically fit to participate in the Event and have no medical conditions that would prevent safe participation. 5. PHOTO AND VIDEO RELEASE Participant grants the Released Parties a perpetual, royalty-free license to use Participant's name, image, likeness, and voice in photographs, video recordings, and other media captured at or in connection with the Event for promotional, educational, and commercial purposes without further compensation or consent. 6. RULES COMPLIANCE Participant agrees to abide by all Event rules, regulations, and instructions issued by officials, coaches, and Event staff. Participant understands that violations may result in removal from the Event without refund. 7. EQUIPMENT AND PROPERTY Participant assumes full responsibility for personal equipment and property brought to the Event. The Released Parties are not responsible for lost, stolen, or damaged property. 8. HEALTH REPRESENTATIONS Participant represents that they have no known communicable diseases and will not attend if experiencing symptoms of illness. Participant consents to any health screening measures required by the Organizer. 9. GOVERNING LAW AND SEVERABILITY This Agreement shall be governed by the laws of the state in which the Event is held. If any provision of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect. The parties agree that any dispute shall be resolved through binding arbitration in the county where the Event is held. 10. ENTIRE AGREEMENT This Agreement constitutes the entire agreement between the parties with respect to its subject matter and supersedes all prior agreements.
